Travel Claims Process

Each claim is different, let us guide you through your claim:

STEP 1

File your claim

The Travel Claims Form must be completed and submitted to Liberty Insurance together with the following supporting documents:

Insurance policy/ certificate (original)
Photocopy of passport
Photocopy of air ticket/boarding pass
Proof of duration of the trip if the travel policy is an annual policy
Other supporting documents specified in section 2 below

What are the required documents to make a claim?
Here are the supporting documents required for each claim section:

ACCIDENTAL DEATH
- Police report (original)
- Death certificate (original)
- Supporting proof such as autopsy report, if any
- Certified copy of the beneficiary's ID card
PERMANENT DISABLEMENT
- Claim Form - Personal Accident
- Medical report (original) (which clearly states the 1st symptom date, medical history, diagnosis etc.)
- Police report (original)
- Detailed description of how the accident occurred and the injuries sustained
MEDICAL EXPENSES AND POST MEDICAL TREATMENT
- Medical documents (Prescription, test result)
- Hospital bills (original)
- Ambulance bills (original)
- Doctor's medical report which states the diagnosis, when symptom(s) first appeared and the patient’s medical history
MEDICAL EXPENSES INCURRED UPON RETURN TO VIETNAM
- Hospital bills (original)
- Medical documents (Prescription, test result, discharge form)
- Doctor's medical report (which clearly states the 1st symptom date, medical history, diagnosis etc.)
- Accident report or immediately advise our Customer Service Center of the injury or sickness upon its occurrence (in case the customer cannot have treatment while overseas)
HOSPITAL CONFINEMENT BENEFITS
Original supporting documents from the hospital stating the period of hospital confinement due to sickness or accidental injury

OVERSEAS HOSPITAL VISIT
- Medical report showing that the Insured is hospitalized for more than 5 days whilst overseas
- Receipt of return economy class air tickets
ADDITIONAL ACCOMMODATION EXPENSES
- Medical report (including medical diagnosis and treatment history) showing that the Insured is hospitalised for more than 5 days whilst overseas
- Receipt/Invoice for hotel charges
TRIP CANCELLATION/ POSTPONEMENT
- Death certificate/ Doctor's certification letter (if the trip cancellation is due to death or serious injury/sickness of the Insured)
- Proof of relationship to the Insured (birth certificate, marriage certificate, etc.)
- Invoice of the amount paid in advance for the trip
- Document from the travel agency confirming the amount refunded
- Invoice showing the cancellation charges paid to the travel agency
TRIP CURTAILMENT
- Purchase receipt for additional ticket
- Letter from the travel agency which shows the breakdown of the portion of the original trip that was not utilised
- Death certificate/ Doctor's certification letter (if the trip curtailment is due to death or serious injury/ sickness of the Insured)
- Proof of relationship to the Insured (birth certificate, marriage certificate, etc.)
- Hotel statement for accommodation expense paid in advance
LOSS OF OR DAMAGE TO BAGGAGE/ PERSONAL EFFECTS
- Property irregularity report if baggage is lost or damaged by carrier
- Police report
- Details of items lost/damaged including date of purchase/actual cash paid, year of make, photo (if any). Attach receipts of items lost (original)
- Quotation/repair invoice/replacement invoice
- Details of amount recoverable from liable carrier or other liable parties
Remarks: Any loss must be reported to police within 24 hours of event

BAGGAGE DELAY
- Travel itinerary
- Property irregularity report from carrier
- Receipt of acknowledgement on returned baggage
LOSS OF MONEY OR TRAVEL DOCUMENTS
- Police report
- Supporting documents for the amount of cash lost (money withdrawal receipt, money exchange slip)
- Receipts for obtaining replacement of travel document
- Hotel bills incurred for replacement of travel document
- Transportation bill
- Administration charges for obtaining replacement of air ticket
Remarks: Any loss must be reported to police within 24 hours of event

TRAVEL DELAY
- Travel itinerary
- Travel documents such as boarding pass/tickets indicating actual departure time & date
- Written confirmation from carrier/their handling agent specifying the reason and hours of delay
TRAVEL MISCONNECTION
- Travel itinerary
- Travel documents such as boarding pass/tickets indicating actual take-off time & date of the onward travel connection
- Written confirmation from carrier or their handling agent on the incoming delay of scheduled public conveyance at the transfer poin
- Original bills/receipts identifying expenses incurred for hotel accommodation, meals and refreshments
Remarks: Transfer point must be outside the location of trip origin

HI-JACKING
Enclosures include written confirmation such as police report or report issued by the common carrier that the Insured Person was a victim of the hijack, including the duration of hijack

RENTAL VEHICLE EXCESS COVE
Certified written copy of Police report, itemised statement of repairs and other documented evidence
